We are currently accepting most major commercial insurances, along with Medicaid, Medicare, HSA plans, and EAP's. If you do not see your insurance provider on the list below, feel free contact us and we will see if we can work out an agreement with them.
- Commercial: Aetna, Anthem, Caresource (Marketplace), Cigna, Humana, Medben, United Health Care
- Medicaid: Aetna, Anthem, Humana, KY Medicaid, Molina to Molina (Passport), UHS Community Plan, Wellcare

Our Case Management Services
Our Case Management Program may assist you with any additional needs that are not listed above. Our case managers work with local organizations to gather and provide resources to our clients.
If you are interested in Targeted Case Management, you will work one-on-one with a case manager who will work with you to identify your needs, develop goals, and work towards achieving those goals.
Our office provides help FREE of charge to qualifying Medicaid recipients.
