What is targeted case management?
Targeted Case Managers work with their clients to work on individualized goals and objectives aimed to be successful in their mental health treatment journey.
Through targeted case management (TCM), we help our clients, both adults and children, gain access to needed medical, clinical, social, and education services with the goal of improving their quality of life.

What areas of your life can a case manager help you with?
We can assist in resources that aids with basic needs such as housing, clothing, and food resources. We can assist with self care routines such as finding and scheduling doctor appointments, gym memberships, and more. We assist with job placement, disability application process, educational opportunities, and government assistance programs. We can also assist with budgeting and financial stability skills.
Children can benefit from case management as well. TCM’s can help with finding community activities, helping with positive behaviors and impulse control, parenting support, support in school, joining school groups and much more.

Daven Mansfield, BS - Targeted Case Manager, Supervisor

Sometimes life’s challenges can be overwhelming. I believe that despite those challenges, everyone deserves the opportunity to live a happy, healthy, satisfying life. All too often the life we envision requires resources and opportunities that are difficult to identify and navigate. My position allows me to reach out a helping hand. As a targeted case manager, I am experienced in providing services including finding employment, obtaining disability or SNAP benefits, behavioral health, medical, and medication management referrals and appointment management, food and housing assistance, educational needs and much more! By working with the client to identify areas of need and develop agreed-upon goals to address those needs, clients will develop skills and utilize resources integral to living the life they envision.

Elizabeth Boehme, BSW - Targeted Case Manager

Sometimes life gets hard and times get tough. Everyone experiences some of that at one time or another. There is a lot of help out there but sometimes it is hard to know who to turn to and where to look when you need some of that help. That is where I come in. Together we will identify your needs and I will connect with appropriate resources in order to meet them. We will work together to get you into a stable situation with housing, food security, utilities, or whatever you may need. It is going to be ok, I am here to help you

Danika Lippert, LPCA - Targeted Case Manager

Life can be overwhelming at times, and everyone could use a helping hand. Whether you need help establishing healthier lifestyle habits or need help applying for services, I would love to help you reach your goals. I can provide you and your family with wrap-around services that help in achieving goals. Some examples of these services include but are not limited to assistance with food emergencies, housing, seeking employment, obtaining disability benefits, obtaining SNAP benefits, and educational assistance. I would love to meet with you and discuss how I can help you.

I have been working in the field for the past 8 years and have also provided individual therapy. Professionally, my passion for social justice and instilling confidence and self-efficacy are foundational elements for working with my clients. I work with both children and adults with issues ranging from substance abuse to linking life enhancing and sustaining resources. In my free time I enjoy spending time with my wife Daven and our three wonderful kids, and rooting for the Reds, Bengals, and Wildcats!

Prior to this, Susie spent a decade at Berea College's Child Development Lab as an early childhood teacher. Now, driven by her passion for serving children and families, she seeks to expand her impact within the community.
