Susan Hight, Targeted Case Manager

Hi, my name is Susan. I serve as a Targeted Case Manager dedicated to advocacy, care coordination, and empowering individuals and families with complex needs. I am currently pursuing graduate degrees in Clinical Social Work (MSW) and Applied Behavior Analysis. My work focuses on identifying strengths, reducing barriers to care, and connecting clients to meaningful supports across home, school, and community systems. With a foundation in Clinical Social Work and behavior analysis, I utilize a trauma informed, culturally responsive, and person centered approach to promote independence, stability, and long term success.
